What is Medication Management?

Health and Wellness Check

Our Senior Care Pharmacist will conduct a health and wellness check which includes measuring blood pressure, blood oxygen levels, blood glucose, and counsel on the appropriate use of medical devices.

Medication List and Reminders to keep you on schedule

A medication list will be formulated on every visit to keep you organized and up to date.  For your benefit, it can be taken to your appointments, so your doctors are informed of medications other doctors and specialists have prescribed.  Unfortunately, medical offices do not communicate, which poses several risks for the patient.  No need to carry all of your medication bottles to your appointments with this simple solution.
